名古屋大学大学院医学系研究科脳神経外科学(齋藤竜太教授)の本村和也(もとむらかずや)准教授と木部祐士(きべゆうじ)大学院生(筆頭著者)の研究グループは、稀な腫瘍である画像上限局型の病巣を形成する IDH野生型組織学的びまん性星細胞種の臨床的および分子生物学的特徴について報告しました。
IDH野生型の神経膠腫は組織学的に低悪性度腫瘍(びまん性星細胞種)の所見であっても膠芽腫と同様に IDH変異型のびまん性星細胞腫と比較して予後不良とされています。このような IDH野生型びまん性星細胞種は脳の広範囲に浸潤するタイプが多く、限局型の病巣を形成することは稀であり、そのような腫瘍の臨床的経過・分子生物学的特徴は明らかになっていません。
本研究では画像上限局型の病巣を形成した IDH野生型組織学的びまん性神経膠腫 5 例の臨床経過・分子生物学的特徴をまとめて報告しました。症例は全て女性で、平均発症年齢は 55.4 歳、発生部位は前頭葉 2 例、島回 3 例でした。初回手術の病理組織学的診断は全てびまん性星細胞腫(grade II)でした。後療法は行わず経過観察をしたところ、わずか平均 12.4 カ月(5.8-28.7 カ月)で再発を認めました。再発時の病理は 4 例が膠芽腫(grade IV)で 1 例が膠肉腫(grade IV)で、全例摘出腔内に限局した造影病変での再発でした。全例再摘出術を施行し、放射線化学療法(Stupp regimen)を施行しました。初回手術検体の遺伝子解析を行い、5 例全例IDH1,2が野生型であることが確認されました。そのうち 4 例で膠芽腫に特徴的なTERT プロモーターの変異を認めました。残りの 1 例でCDKN2A homozygous deletionを認め、再発までの期間は 5.8 カ月と最も短期間でした。これらのことから、限局した病変で病理所見上の特徴は低悪性度神経膠腫と酷似していても、IDH野生型びまん性星細胞種は非常に予後不良であり、分子生物学的解析による診断が重要であると考えられます。

山口純矢先生、大岡史治先生が筆頭著者の脳腫瘍関連の論文がCancer Medicineに掲載され、プレスリリースを行いました。
名古屋大学大学院医学系研究科脳神経外科学の山口純矢 医員、大岡史治 講師、齋藤竜太 教授の 研究グルーフは、85 症例の中枢神経系原発悪性リンパ腫(PCNSL)の遺伝子異常と予後の統合解析を行い、CD79B Y196 変異は、PCNSLに対するR-MPV療法 の良好な反応性の予測因子になることを同定いたしました。また、PCNSLで特異的、かつ高頻度にみられるMYD88 L265P変異を診断マーカーとして、MYD88 L265P変異とCD79B Y196変異を同時に、かつ迅速に解析するシステムを開発し、PCNSLの分子診断と治療反応性の予測を90 分以内で判定することが可能となりました。 PCNSL は高齢者に好発する稀な悪性脳腫瘍のひとつですが、近年の高齢化に伴いその発生数は増加しています。化学療法の発達に伴い、PCNSLの予後は改善してきておりますが、その希少性からまとまった症例数での長期的な予後や予後因子に関したデータは十分ではありませんでした。そこでR-MPV 療法で治療を受けた85 症例のPCNSL の遺伝子異常と予後の解析を行い、R-MPV療法は従来のHD-MTX療法に比較して長期予後に優れ、またCD79B Y196変異を持つ症例では、R-MPV療法の感受性が良く、予後が良好であることがわかりました。このCD79B Y196変異による予後改善効果は、HD-MTX 療法を受けた症例では観察されず、CD79B Y196 変異はR-MPV療法の良好な反応性の予測因子であることがわかりました。さらに、MYD88 L265P 変異を診断マーカーとして、MYD88 L265P変異とCD79B Y196変異の同時迅速解析系を開発し、PCNSL の分子診断と治療反応性の予測を、少量の腫瘍検体から90分以内に判定することが可能になりました。 本研究で得られた成果により、PCNSLの迅速、かつ正確な分子診断、CD79B Y196 変異の有無を軸にした治療の層別化が期待されます。

World`s first experiment model of meningioma was established by Nagoya University group. This patient derived organoid model will pave ways to better pathological analysis leading to novel therapies.
Graduate student Shintaro Yamamoto, associate professors Fumiharu Ohka and Atsushi Natsume`s research group from Nagoya University (Research Dean Kenji Kadomatsu; Neurosurgery Chairman Ryuta Saito) successfully established a novel meningioma organoid model from patient derived tumor. Meningioma is the most common primary brain tumor. While the standard therapy including surgery and radiation usually bring favorable outcomes, malignant cases and incomplete resection even in benign cases may recur multiple times leading to bad survival prognosis. Although recent research has led to identification of molecular abnormalities associated with tumor proliferation, no molecular targeting drugs have been developed dues to the lack of elucidation of tumor proliferation mechanism. So far it had been difficult to conduct pathological analyses due to the lack of proper cell line and animal models for research. Recently, organoid models using various 3D cancer culture has received limelight as promising research models. We were able to establish meningioma organoid models from all the cultures in every case using this culture technique for the first time in the world.
We focused on the abnormal expression of FOXM1 gene in rapidly growing malignant meningioma and verified the influence of FOXM1 in tumor proliferation using the meningioma organoid model. We identified the change in tumor proliferation in the organoid model by altering FOXM1 gene by over expression or knockdown and demonstrated the potential of FOXM1 as a novel therapeutic target by inhibiting tumor growth using a combination of FOXM1 inhibitor and radiation therapy. This experiment model is expected to aid in the elucidation of detailed pathology of meningioma and development of novel therapeutic strategies. The results of this study were published online in “Neuro-Oncology” on July 2, 2021.

Elucidation of optimal therapy to prevent malignant transformation in brain tumor- optimal treatment strategies tailored for each case revealed by mathematical modelling.
Dr. Kosuke Aoki (Specially designated assistant professor), Atsushi Natsume (Associate professor) of Nagoya University graduate school of medicine Department of Neurosurgery along with Dr. Haeno Hiroshi (Specially Designated Associate Professor) of Tokyo University revealed the optimal therapeutic approach to prevent malignant transformation of low grade IDH-Mutant Gliomas (Grade II) using mathematical analysis.
Diffuse glioma, one of the most common brain tumors, is refractory to treatment. Low grade IDH mutant gliomas, an initially slowly growing tumor common among people in their 30s and 40s, becomes life threatening after malignant transformation. Surgical resection along with chemotherapy and radiation therapy control tumor growth. However, these therapies are also reported to trigger malignant transformation. The optimal timing of treatment to prevent malignant transformation is yet to be understood.
In this study, we performed a comprehensive gene mutation analysis and constructed a mathematical model for tumor progression based on treatment history and MR Images of 276 low grade glioma patients from 10 different institutions in Japan.
These analyses revealed that the number of tumor cell genetic mutations is strongly associated with the risk of malignant transformation and that postoperative treatments such as chemotherapy and radiotherapy suppress tumor growth while increasing the risk of malignant transformation per cell. Moreover, we were able to show that the ideal treatment to prevent malignant transformation differs for each case. We found that starting chemotherapy and radiotherapy immediately after surgery is the optimal treatment to prevent malignant transformation in small tumors (less than 50cc in volume at the time of first surgery). On the other hand, while chemotherapy and radiation immediately after surgery is the optimal treatment in most cases of larger tumors (more than 50cc), postoperative treatment accelerates malignant transformation in cases with specific gene mutation patterns and with insufficient surgical resection. Moreover, we were able to demonstrate that early diagnosis and early treatment are extremely important in preventing this malignant transformation. The result of this study is a very important finding that has clarified the optimal treatment strategy for each patient to prevent malignant transformation of low-grade IDH mutant gliomas and improve survival.

Elucidation of mechanism of change in interoception and emotion before and after brain tumor surgery: Interdisciplinary research of brain and mind neurosurgery and cognitive neuroscience.
Research group of Professor Satoshi Umeda and Dr. Yuri Terasawa (Associate Professor) of Keio University Department of Psychology and Dr. Kazuya Motomura (Associate Professor, Nagoya University Department of Neurosurgery) tested the emotion recognition ability of brain tumor patients before and after tumor removal and showed that the decline in this ability is associated with a decline in the ability to perceive changes in the internal state of the body (interoceptive sensation).
Although various studies have been conducted on the relationship between the function of the brain and the mind, how the perception of one's own emotions, such as happiness, sadness, and anger, changes when an actual brain region is damaged or removed remains unknown. From the results of previous research, it has been suggested that stimulation and removal of the insular cortex (insular gyrus), which is closely related to the perception of changes in the body's internal states such as heartbeat and respiration, leads to changes in the recognition of excitatory emotions such as anger. In this study, In this study, we examined the role of the insular cortex in cardiac interoception and recognizing emotions from facial expressions (task involving recognition of facial expression from photographs of faces) by comparing them pre- and post-operatively in 18 patients with tumors involving the insular gyrus. Comparison of the results of these tasks before and after surgery demonstrated significant associations between the changes in interoceptive accuracy and sensitivity to expressions of anger and happiness. This suggests that the insular cortex functions as a neural substrate for interoceptive sensations, which are information from within the body and supports the recognition of emotions such as anger and joy. It also suggests that the bodily sensations such as throbbing and nervousness are essential for experiencing rich emotions. It can also be conceived from this study that traumatic and age-related changes in the region in and around the insula may also change perception of emotions.
